To which coconut milk are recipes referring-the creamy stuff in a can or the milk substitute?

I think of coconut milk as the stuff in a can, very creamy. However, I am wondering if you mean the milk substitute that I have started to see in the milk aisle of late alongside the Almond milk and Rice milk.

We are often referring to the milk substitute. The milk in the can is often higher in fats and too thick to add to a smoothie.
